Community Scholarship Application

Community Scholarship Application Deadline December 4, 2019 by 4:00 p.m.
This application is completed and submitted online through the link above. You must set up an account before you can access the application. Once set up, you can begin the application & save it as you go.

Parent Statement/Signature Page Deadline December 4, 2019 at 4:00 p.m.
You are also required to complete and print out a copy of the Parent Statement/Signature Page (which can be downloaded here: Parent Statement DocX Version or Parent Statement PDF Version). Your parent can make a statement here on your behalf – a statement is highly encouraged, but not required. You must sign it, your parent/guardian must sign it (even if they do not make a statement), and you must upload it to your application. Once uploaded, you must submit the completed form with the original signatures of you and your parent/guardian to Mrs. Whitson by 4:00 p.m. on Wednesday, December 4. APPLICATIONS WITHOUT THE PRINTED AND SIGNED SIGNATURE PAGE SUBMITTED BY THE DEADLINE ARE NOT ELIGIBLE. 

TRANSCRIPTS:
You are required to upload of a copy of your 6-semester transcript to the application.  Mrs. Whitson will get a digital copy of your transcript - she will email you a PDF of it or print you a copy that you will upload to your application.


ACT SCORES:
You are required to upload a copy of your highest ACT score report. Access your highest ACT score from the ACTstudent.org website and click "Download Student Score Report PDF" (top right) to save a copy of your ACT score report.

A RECENT PICTURE:
You are required to upload a recent, *school appropriate* picture of yourself. It can be a selfie, a Senior picture, whatever you like - do not post a group shot, because a donor will not know who you are in the picture. 


IMPORTANT:
Please remember that completing the application does not mean that you have applied for a scholarship. You MUST check the scholarship list (on the HHS Scholarship Website, via Remind or in the counseling office) for scholarships that you qualify for and then tell Mrs. Whitson you wish to apply for the scholarship.  If you do not tell Mrs. Whitson to submit your application, your application will not be submitted!
